Reminder NYS requires a physical exam for new entrants and students in grades pre-K or K, 1, 3, 5, 7, 9, and 11. NYS also requires physical exams annually for interscholastic sports, working papers, and as required by the Committee on Special Education (CSE). The date of examination must not be more than 12 months prior to the start of the school year and noted on the form. Immunizations should be updated yearly for all students.
Please keep your school nurse updated on your child's health status, such as surgeries, medications, illness/diseases, new diagnoses or other conditions that may affect your child's school performance. Please make sure the emergency card is completed, signed and updated as needed.
